How to Apply for a Contribution
If your contribution request meets Wisconsin Public Service Foundation guidelines, complete our application form, or send an application package that includes:
- A one-page cover letter identifying your organization; the name, title and telephone number of a contact person; the purpose of the request; and the specific dollar amount requested.
- A brief narrative (no more than 3 pages) describing the background and
objectives of the program or project, including a description of:
- Your organization, it's purpose, population served and geographic area of service.
- The program or project, anticipated time line, expected outcomes and your organization's capacity to manage the program or project.
- How the program or project fits with Wisconsin Public Service Foundation's funding interests.
- A current-year operating budget and project budget. (This should list funds raised as well as identify other revenue sources and their status.)
- A list of the organization's governing board of members and their professional affiliations.
- A list of any employees or retirees of your local utility who are involved in the fund drive or organization.
The following items are also needed for requests of more than $500:
- The organization's audited financial statement for the most recent fiscal year.
- IRS form 501(c)(3) tax determination letter.

Proposal Review
Applications are accepted and reviewed on an ongoing basis. Decisions on applications are generally made within four months of submission, depending on the request. Every application submitted in writing will receive a response.
